高麗女子奇承梁(河智苑飾)，兒時與母親淪為貢女被進獻給元朝，途中母親被元朝武將殺死，無家可歸的她女扮男裝隱瞞身份，成為瀋陽王王暠(李在勇飾)手下。承梁長大後，結識了高麗世子王裕(朱鎮模飾)並互生好感，承梁更奉王裕之命，保護身處大青島的元國皇太子妥懽(池昌旭飾)。承梁拼死保全妥懽性命，卻被妥懽間接害死了她父親。
王暠為謀奪皇位，與元朝合作陷害王裕，令其廢位並流放；而承梁亦因而被識破女兒身，被派去元朝皇室當貢女，心中一直不忘向已成王的妥懽復仇。未料承梁意外捲入皇后答納失里(白珍熙飾)計謀中，經歷九死一生後，她決心報仇，與命運抗爭。
承梁以妥懽才人的身份再次入宮，並利用妥懽對她的感情，一步步攀上高位，但與此同時，她對妥懽的感情亦起了變化。經歷了連番的宮廷鬥爭，她終於登上元朝皇后之位，並以其聰明才智策劃政治計謀，成為史上頗具影響力的奇皇后。
